Enrollment Application

YogaBDSM and Yogi Dominanda require that all potential students complete the Enrollment Application.  These questions are designed help us at YogaBDSM identify and customize your specific practice needs and goals.

Please answer the questions using the scale provided or if giving a text answer, simply type YES or NO.  If answering YES to a health question please provide additional details that may be helpful.  Please complete all questions to the best of your ability as incomplete applications will be rejected, you can always change an answer if necessary.
